背景:近年來有許多研究發現極高濃度(≥90mg/dL)的高密度脂蛋白膽固醇與全死因、心血管疾病、癌症的危險性有關。由大規模觀察型研究指出在低濃度以及極高濃度的高密度脂蛋白膽固醇的情況下,心血管疾病的死亡風險大於適當濃度的高密度脂蛋白膽固醇;此外也有許多臨床試驗的研究使用膽固醇酯轉移蛋白(cholesteryl ester transfer protein,CETP)抑制劑進行介入,以提升高密度脂蛋白膽固醇的濃度,然結果發現心血管疾病的死亡風險並無下降。至今其中的機制仍然不清楚,因果關係有待證實。因此本研究的目的為利用雙樣本孟德爾隨機分派試驗來探討台灣人極高濃度的高密度脂蛋白膽固醇與心血管疾病

的因果相關性。研究方法:本研究採用雙樣本的孟德爾隨機分派試驗方法,研究對象均來自於2008年收案至今的台灣人體生物資料庫,再排除了無基因資料、人口學變項、生化檢驗值、共病症資料後,剩餘91,151人納入研究。依照收案時間、性別比例平均分為兩組:樣本一為45,575人以及樣本二為45,576人。利用樣本一資料進行全基因組的關聯性分析,找出與高密度脂蛋白膽固醇顯著相關的單一核甘酸基因多型性(single nucleotide polymorphism,SNP)位點,加權計算成多基因風險分數來當作工具變量。在樣本二中先進行工具變量與極高濃度的心血管疾病風險之相關性分析,並利用兩階段最小平方法來進行線

性孟德爾隨機分派試驗分析,以及利用分數多項式法來進行非線性孟德爾隨機分派試驗分析。結果:本研究結果顯示與高密度脂蛋白膽固醇顯著相關的SNP位點有15個,均集中在第9、16號染色體上,分別坐落於CETP、ABCA1基因。與適當濃度的高密度脂蛋白膽固醇組別(40~59 mg/dL)相比,極高濃度高密度脂蛋白膽固醇組別(≥90 mg/dL)發生血管疾病的危險對比值(odds ratio,OR)為1.34 [95%信賴區間(confidence interval,CI)為1.06~1.70],達統計上的顯著差異。線性孟德爾隨機分派試驗的結果顯示,對於總體樣本而言,隨著高密度脂蛋白膽固醇的單位濃度增加,

本研究主要目標是探討一烷化劑即甲磺酸甲酯(methyl methane sulfonate, MMS)及氧化劑即過氧化氫(hydrogen peroxide, H2O2)於小牛胸腺 DNA (CT-DNA)誘導易致突變 DNA 損傷之特定缺鹼基核酸位點(abasic sites) 之形成,並進行結構特徵分析,比較特定類型缺鹼基核酸位點之差異。濃度-效應實驗研究結果顯示,小牛胸腺 DNA以 H2O2 (0、0.1、1 和 20 mM)反應處理後,缺鹼基核酸位點之背景值(平均值±標準偏差)推估為 38.7 (± 4.4)、47.1 (± 6.2) 、61.4 (±1.4) 和 171.8 (±5

.8) (AP sites/106 總核苷酸),而以MMS(濃度為 0、0.1 和 0.5 mM)反應之CT-DNA 其缺鹼基核酸位點背景值估計為 37.2± 3.0、42.1 ±2.6 、 49.0 ±3.9 (AP sites/106 總核苷酸)。時間依賴生成缺鹼基核酸位點反應實驗結果CT-DNA與 H2O2 反應後,缺鹼基核酸位點之背景淨值逐漸上升,反之,與MMS 反應後之缺鹼基核酸位點之背景淨值逐漸下降,此一結果可轉譯成MMS所誘發之缺鹼基核酸位點可能為具化學不穩定性。進一步的研究顯示,經由甲氧胺 (Methoxylamine, MX) 預處理 CT-DNA可顯著降低與 H2O2 或

MMS 反應後缺鹼基核酸位點之背景值之形成。依據缺鹼基核酸位點結構特徵分析實驗顯示,若以腐胺切割試驗(putrescine cleavage assay)經由誘導β-elimination以切除5’端或3’端斷裂之缺鹼基核酸位點。結果顯示,於 H2O2 (20 mM) 及 MMS (0.5 mM) 反應後 CT-DNA 中腐胺可切除之5’端或3’端斷裂之缺鹼基核酸位點,比例估計分別為 76% 和 7.0%。此外以 T7 Exo 及 Exo III 外切酵素(exonuclease)進行切割實驗,探討缺鹼基核酸位點之結構特徵,再次驗證由 H2O2 (0.1 mM) 誘導缺鹼基核酸位點,大多數

是具有5’端或3’端斷裂之缺鹼基核酸位點,且未發現結構完整缺鹼基核酸位點 (intact AP sites) 。然而相反地,MMS (0.1 mM) 可誘發結構完整缺鹼基核酸位點,數量估計為 1.4 AP sites/106 總核苷酸, MMS 反應生成之 缺鹼基核酸位點不能被腐胺及 (T7 Exo + Exo III) 切除,並且被歸類為結構完整缺鹼基核酸位點。總體而言,上述之證據進一步支持以下論點,亦即氧化壓力 (oxidative stress) 是誘發單鏈斷裂串聯缺鹼基核酸位點形成的主要驅動力。
