台灣末期腎臟病之盛行率為全球之冠,腎臟功能衰竭的患者,會惡化為尿毒症 (Uremia)。尿毒症狀雖能藉由血液透析改善,但尿毒症引起的併發症 (如:皮膚搔癢),仍會對生活品質造成影響。本臨床研究針對血液透析患者體內的慢性發炎狀況,介入富含乙型轉化生長因子 (Transforming growth factor beta 2, TGF-β2) 的乳清蛋白萃取物,探討TGF-β2對於活化患者體內調節型T細胞及調節發炎與皮膚搔癢的關聯性。本研究於輔仁大學附設醫院及新店耕莘醫院,招募九十四位血液透析患者,每日給予800毫克乳清蛋白萃取物持續三個月。分別於介入前、後,進行免疫功能與發炎指標的分析,並藉由

皮膚搔癢及焦慮憂鬱問卷調查,和收集常規監測生化數值之統整結果作為評斷指標。結果顯示,血液透析患者介入三個月後,血漿TGF-β2濃度明顯增加。此外,我們觀察到血液透析患者介入乳清蛋白萃取物三個月後,其皮膚搔癢量表 (Visual Analogue Scale, VAS與5D itch scale) 介入後,相較於介入前確實有明顯降低。但血漿中高敏感度C-反應蛋白 (High-sensitivity CRP, hs-CRP) 及介白素6 號 (Interleukin-6, IL-6),並無顯著下降。在周邊淋巴球次分群的部分,考量疫苗注射可能會影響免疫細胞百分比,因此將疫苗接受與否的干擾因子進行校正

,結果顯示,血液透析患者介入三個月的專利乳清蛋白萃取物後,周邊血的毒殺型T細胞百分比明顯上升且B細胞的百分比有明顯下降且達顯著差異。在體外培養的吞噬活性檢測中,觀察到顆粒性白血球及單核球的活化程度,介入後明顯降低,但在周邊血中的顆粒性白血球、單核球及淋巴球百分比,介入前、後並無明顯的改變,且未觀察到吞噬活性之降低與血漿TGF-β2濃度增加之間的相關性。對於顆粒性白血球的吞噬活性改變,可能是活化吞噬作用的訊息傳遞路徑受到影響,但仍須透過進一步的體外培養實驗釐清當中的影響機制。最後,將血液透析患者的周邊血液單核球進行體外培養,以了解調節型T細胞在體內的百分比變化。結果顯示,血液透析患者介入三個月專

利乳清蛋白萃取物後,其周邊血液單核球在PHA刺激下,調節型T細胞的百分比增加倍數,明顯高於介入前的調節型T細胞增加倍數。綜上所述,給予血液透析患者三個月的乳清蛋白萃取物,確實能增加血漿TGF-β2濃度,調節型T細胞的表現量也在刺激後明顯增加,同時有助於降低皮膚搔癢分數而改善生活品質。
